USPS Form 1583 is a form that authorizes a Commercial Mail Receiving Agent to open your mail and forward it to you.
Commercial Mail Receiving Agents (CMRA)
A CMRA is a business that you authorize to handle your mail. It is generally against federal law to open someone else’s mail or to re-mail unopened mail. However, you can authorize a CMRA to open your mail, forward it, scan it, or dispose of it as you choose.
You can use a CMRA for privacy, convenience, or a home for your business. You can use a CMRA to verify your address for Google, payroll taxes, business nexus, and more. A CMRA gives you a real address in a real location.
Security and Form 1583
In order authorize a CMRA to handle your mail, first you must prove that it is actually you that is authorizing the opening of your mail. A Notary Public is a person who can legally verify your identity. USPS Form 1583 needs to be signed in front of a notary. A Notary verifies your identity. This prevents fraud and identity theft. A CMRA cannot open or forward your mail without a notarized USPS Form 1583.
Why Do I Need a New Form Now?
Since I moved my location, the mail form is now incorrect. Everyone needs an updated Form 1583 showing the corrected address where you are receiving mail. Get a blank form HERE or you can contact us at diane@dianehagerty.com and we will email you a pre-filled form.
You will need a separate form for each person receiving mail. You can include your business on the same form as you. Once you complete the form, send it to me via email or mail.
Do I have to Pay $25 again?
No. You can print out the form and take it to any Notary Public to be notarized. You are not required to use Anytime Mailbox’s online notary service. Most banks have several notaries available.

What Do I Do with the USPS Form 1583?
Once you have the new form notarized, then you need to get it to ME. The easiest way to get it uploaded is to email it to diane@dianehagerty.com and I can upload it to your customer file. You can also mail it to me if you don’t have the ability to scan.
